Physician’s
Recommendation
Patients need their physician to create a patient certification with the SD Department of Health to start the card application. When the physician completes the certification, the patient will get an email from noreply@sd.airlift.app
Start Your
Application
Required Documents
1) SD Driver’s License, Passport, Tribal
ID or SD Student ID Card
2) Passport Photo
May need wage stubs, tax return, and proof of income
Custodial or Legal Parent Attestation and Designated Caregivers will have additional fields to fill out.
Shop or Grow
The state application will give you the opportunity to register to grow at home or shop at a dispensary. It’s not required to select a dispensary to complete the application — True North Dispensary would be happy to be your dispensary of choice!
Pay Up
Application Fees:
$75 - Initial application and yearly renewal
$20 - Low-income application and renewal
$20 - Home cultivation registration
$20 - Additional caregivers
Submit
your application
Follow your application information on your patient dashboard. Once the application is reviewed and approved, the Department of Health will mail the card to the patient.
